PO-05-043 RIGHT VENTRICULAR POSITIONING OF LEADLESS PACEMAKER MAY INFLUENCE LEFT VENTRICULAR ELECTROMECHANICAL SYNCHRONY

چکیده
Leadless pacemakers (LP) reduce certain complications of conventional pacemakers, yet limitations single chamber right ventricular (RV) pacing remain, including pacing-induced dyssynchrony. The Micra LP is positioned in RV septum, avoiding the free wall to minimize perforation, and location primarily determined by capture threshold sensitivity. Whether positioning mid upper septum vs. apical affects degree left (LV) synchrony be determined. We sought establish whether position on LV synchrony, using strain imaging speckle-tracking echocardiography. Between May 2019 February 2022, 53 consecutive patients implanted with were screened. Of those, eleven found 100% paced have an adequate quality transthoracic echocardiogram (TTE) for analysis. Chest 12-lead electrocardiogram (EKG) used identify implantation site (apical high septum). focused views processed Tomtec software acquire variables ventricle mechanical dispersion (LVMD) maximal opposing delays from 18 segment model. QRS duration (QRSd) was obtained EKG. Mann-Whitney U test Fisher’s exact compare independent groups. All analyses performed SPSS Ver 29.0 (IBM Corporation, Armonk, NY). Electrical dyssynchrony measured QRSd showed a tendency longer septal group (n= 5) compared higher 6), but it not statistically significant (159.6 +/- 13.8 148.33 16.15, p = 0.302). Ejection fraction no difference between two groups (median 64 60, 0.879). Degree mitral regurgitation also (U 11.5, 0.537). Higher placement resulted improved as measure lower LVMD shorter delay 2 3, 0.017 0.03 respectively). Implantation displays speckle tracking derived strain, which positioning. This may consequences cardiac function requiring continuous pacing. Larger studies are needed determine these preliminary findings generalized larger population.

Acute effects of right ventricular apical pacing on left ventricular synchrony and mechanics.
BACKGROUND Chronic right ventricular (RV) apical pacing has a detrimental effect on left ventricular (LV) function. However, the acute effects of RV apical pacing on LV mechanics remain unclear. The purpose of the study was to assess the acute impact of RV apical pacing on global LV function, evaluating LV contraction synchrony and LV shortening and twist, using 2D speckle-tracking strain imagi...
متن کاملLate deterioration of left ventricular function after right ventricular pacemaker implantation
OBJECTIVES Right ventricular (RV) pacing induces a left bundle branch block pattern on ECG and may promote heart failure. Patients with dual chamber pacemakers (DCPs) who present with progressive reduction in left ventricular ejection fraction (LVEF) secondary to RV pacing are candidates for cardiac resynchronization therapy (CRT). This study analyzes whether upgrading DCP to CRT with the addit...
متن کاملInfluence of right ventricular stimulation site on left ventricular function in atrial synchronous ventricular pacing.
OBJECTIVES The study investigates the correlation between left ventricular function and QRS duration obtained by alternate right ventricular pacing sites. BACKGROUND 1. Right ventricular apical pacing is associated with alterations of left ventricular contraction sequence. 2. A stimulation producing narrow QRS complexes is supposed to provide for better left ventricular contraction patterns. ...

A62-year-old woman with a history of vasospastic angina presented to the emergency room with chest pain. The ECG showed ST-segment elevation in leads V3 through V6. The transthoracic echocardiogram revealed an akinetic left ventricular (LV) apical wall. The coronary angiogram on admission demonstrated normal coronary arteries. Two weeks later, resting Tc-tetrofosmin myocardial single photon emi...
متن کاملRight ventricular size assessed by cardiovascular MRI may predict mortality after left ventricular assist device placement
Methods Patients were referred for cardiovascular magnetic resonance imaging prior to LVAD placement. We assessed the association of mortality to pre-LVAD right ventricular end systolic volume index (RVESVI), right ventricular end diastolic volume index (RVEDVI), left ventricular ejection fraction (LVEF) by CMR. Right ventricular stroke work index (RVSWI) was determined by preLVAD right heart c...
